Participants, parents and guardians must recognize that there is an inherent risk of injury when choosing to participate in program activities. Hansa Soccer Academy continually strives to reduce the risks and insists that all participants follow safety rules and instructions designed to protect participants’ safety. Failure to comply with Hansa Soccer Academy’s established program rules may result in the participant’s removal from the program. Hansa Soccer Academy does not carry medical insurance for injuries or illness sustained in this program. Participants and parents should review their own health insurance policy for coverage.
As the parent or guardian of the participant in the Hansa Soccer Academy program, I recognize and acknowledge that there are certain risks of physical injury. I agree and relinquish all claims against Hansa
Soccer Academy and their staff that I may have as a result of any and all injuries, damages or lossesthat my child or I may have or which may accrue to me through participation in the program. I further agree to indemnify and hold harmless and defend the Hansa Soccer Academy and their staff from any and all claims resulting from injuries, damages and losses sustained by my child, or me arising out of, connected with, or in any way associated with the activities with the program. In the event of an emergency, I authorize officials of Hansa Soccer Academy to secure from any licensed hospital, physician, and/or medical personnel any treatment for any immediate care and agree that I will be responsible for payment of any and all charges for medical services. I have read and fully understand the above program details, waiver and release of all claims and permission to secure necessary
PHOTO WAIVER AND MODEL RELEASE :
I hereby release, discharge and agree to let Hansa Soccer Academy, the use of photographs and video taken at the camp to be produced for our print collateral, yearly video or web page, to be displaying of said picture or images, and from liability for violation of any personal or proprietary right that I may have in conjunction with said pictures or images and with the use thereof.
REFUND POLICY:
Please note that you can cancel or transfer to another course/camp date if you notify us 10 days or more in advance of the start date. We will refund your payment less a $50 service fee. Otherwise cancellation for any reason is subject to 100% of the course/camp fees. These courses/camps are strictly non-transferable to another child.
The camp closes at 4pm so pick up time is 4PM sharp. If you know you will be late you must contact the camp director 24-hours BEFORE the late pick-up to confirm whether it is possible to have staff available to work for the additional time ( This e-mail address is being protected from spambots. You need JavaScript enabled to view it , 773.366.6640). There will be a fee charged for this after care.
The Hansa Soccer Camp ends at 4:00PM each day. Due to late pick ups a late fee policy has been established. Please understand that when a parent/guardian is late picking up their child, we must have at least one instructor remain at the site for the safety of your child. These instructors must be compensated for their time and this can ultimately cause an increase in our program fees for everyone.
Effective June 19, 2012 the following guidelines will be followed:
-A $20 late fee (per child) is charged for the first 15 minutes after the program closes.
-An additional late fee of $20 will be charged for the next 15 minutes. For example, if a parent arrives at 4:16PM, he or she will incur an additional $20 late fee, total fee is now $40 (per child).
-If a parent arrives after 4:30PM, the fee will increase with a $20 charge (total charge for being 45 minutes late would be $60)
-***Late fee is to be paid immediately upon your arrival;failure to pay can result in termination from the program.
-If after 4:45pm Hansa instructors have not been contacted and are unable to reach a parent or guardian who is available to pick-up your child, the township police department may be contacted.
-Time is determined by the time on the camp director's iPhone.
